Support At Home

For carers

Support at Home provides personalised services that support older people to maintain their independence, wellbeing and quality of life, enabling them to continue living safely and confidently in their own home.

Services are tailored to each person’s goals, preferences and circumstances and may include:

  • Clinical Supports: Such as nursing and allied health services.
  • Independence Supports: Including personal care, social support, community participation, respite, transport and assistive technology
  • Everyday Living Supports: such as domestic assistance, home maintenance, gardening, meal preparation and meal delivery
