Home Care Packages provide case-managed supports to older people with complex ageing related needs to enable them to continue to live in their own home.
Services provided through Packages are classified as:
- clinical services: nursing, allied health
- independence services for activities of daily living: personal care, social support, community engagement, respite, transport, and assistive technology
- everyday living supports: domestic assistance, home maintenance, gardening, meal preparation and meal delivery.
Entry to the program is via MyAgedCare (myagedcare.gov.au) or call 1800 200 422. An assessor will meet with you to discuss your goals and support needs and recommend the services that will be funded by the package.
